Wisconsin Blues Music Hall Of Fame

Use this form to nominate a musician, band or non-performer living in (or having roots in) the state of Wisconsin that performs, promotes or supports blues music and has made a substantial contribution to the blues scene. Enter one nomination per submission. It is not required that a musician, band or non-performer is still active in the scene today. All those nominated will be considered for induction into the Wisconsin Blues Music Hall of Fame.
